| Your Disability Evaluation Session |
PROFESSIONAL RELATIONSHIP For military disaiblity evaluations, Dr. Criswell is an “independent contractor” for QTC, Inc. QTC conducts disability evaluations for the Department of Veterans Affairs. For a military disability evaluation, your relationship to Dr. Criswell is claimant-evaluator. The claimant is not charged for the evaluation session. You will not become a patient of Dr. Criswell’s for this evaluation even though the evaluation will take place in Dr. Criswell's office. There is no treatment service provided in the military disability evaluation.
Dr. Criswell can not conduct a disability evaluation on anyone she has worked with in a doctor-patient relationship.
EVALUATION PROCESS Dr. Criswell conducts the evaluation by reviewing files provided by QTC and by talking with the claimant during one, in-person session.
She will follow a structured interview protocol to meet VA requirements for the evaluation. It could possibly be upsetting to review what you have been through. Dr. Criswell is experienced in working with military personnel. Dr. Criswell will report findings to QTC in a timely manner. The information is not confidential to Dr. Criswell. The VA is the “owner” of the disability evaluation. Dr. Criswell can not provide results of the evaluation to the claimant. Claimants are welcome to contact the QTC Case Coordinator with any questions. The disability rating will be provided to the claimant by the VA.
DIRECTIONS TO OFFICE Please see "Office Info" tab, "Directions." QTC provides directions to the claimant as well.
APPOINTMENT TIME Please be on time! Your session will very likely start at the appointment time; if there is a wait, it will be very short. Emergencies could cause a wait, but this is unlikely, and you'd be informed. If you will be late or need to reschedule, please contact your QTC Case Coordinator. Dr. Criswell is not allowed to reschedule appointments. It has to be done through QTC.
PARKING It is a good idea to allow some extra minutes for parking in case the lot is full. There is a small parking lot at the building. You can park anywhere except in reserved spaces. If the lot is full (it happens), park on the street.
Street parking near the building is available most of the time. There are quarter-meters on S. Washington Street in front of the building. There is Free, 2-hour parking on the side streets around the building: Wolfe, Duke, St. Asaph, also on S. Washington Street in the next block away from the building. Occasionally, someone will need to park more than a block away, but usually parking is available near the building.
Two hour parking should be more than enough time.
METRO The King Street Metro is just under a mile away. If you are up for an interesting walk through Old Town or can take a short taxi ride, take the Metro.
WHEELCHAIR ACCESS Wheelchairs access the building through the front door on Washington Street.
There is one handicapped parking spot; it is the first space in the lot by the front of the building. If you need handicapped parking, please let QTC know so that information can be passed on to Dr. Criswell in advance.
Wheelchair accessible restroom is on the 5th floor, in the Old Town Dental suite. Dr. Criswell can show you where it is.
RECEPTION Dr. Criswell does not have office staff. Just make yourself comfortable in the waiting room. You do not fill out anything while you are waiting; you will not "register" with Dr. Criswell's practice. Dr. Criswell will meet you at the appointment time. Restroom key is in the waiting room; restroom is in the hall outside the suite.
EMERGENCIES If any emergency situation arises during the evaluation session, Dr. Criswell will help the claimant as best she can, and take whatever measures are necessary, for example, calling 911 if necessary. She would also notify the QTC Case Coordinator in Philadelphia.
If it looks like the claimant is at risk for self harm or harm to others, even though not in an emergency situation, Dr. Criswell will notify the QTC Case Coordinator for follow-up.
